453.4 - MEDICATION AT SCHOOL

Prescription medications require a written consent from physician and parent/guardian. The consent will contain the date, name of student, name of medication, dose, route or site of application, and time to be given. No prescription medications will be given with verbal consent. The signed consent forms will be kept in the student's health folder. Prescription medications must be brought to school in the original marked or labeled container.

Non-prescription medications require written consent from the parent/guardian. The consent will contain the date, name of student, name of medication, dose, route or site of application, and time to be given. The signed consent forms will be kept in the student's health folder. Non-prescription medications will be given with verbal consent from the parent/guardian and followed with written verification of consent from the parent/guardian. Non-prescription medications must be brought to school in the original container.

All medications given are documented on a medication daily log sheet by the provider who administered the medication. This includes the student name, name of medication, dose, route or site of application, time medication was administered, and initials of person that administered the medication. The medication is administered only by the medical assistant and administration designated others. All medications are to be stored in a safe area and locked daily. Medication containers will be stored in the office store room in the high school and at the health office in the grade school.
