448 - EMANCIPATED STUDENT
WHEREAS, the parties, ________________________________________, hereafter known as "Student" and ____________________________________, Principal at Auburndale High School, hereafter known as "School" are desirous of contracting for the purpose of entering into an agreement outlining the duties and responsibilities of the respective parties in the provision of educational services to student;WHEREAS, student has reached the age of majority and has declared him/herself emancipated for the purpose of monitoring school attendance;
WHEREAS, Auburndale High School has enacted an attendance policy applicable to all students who attend Auburndale High School;
WHEREAS, student has been advised of and has received a copy of the Auburndale High School attendance policy and student code;
NOW THEREFORE, IT IS HEREBY STIPULATED AND AGREED AS FOLLOWS:
Students shall not be absent for more than one day for every ten school days;
Should student be absent for two or more days, student shall provide a note from a licensed physician which indicates that the student should not attend classes. Student agrees that he/she will be considered unexcused should he/she not provide this note after two days of absence;
Student agrees that five or more absences in a nine week grading period or ten or more absences in a semester period, results in loss of credit for all classes;
Student further agrees that absences shall be defined to include full or partial non attendance of classes and suspensions;
Student further agrees that he/she shall not be tardy at the commencement of the school day and agrees that if he/she is tardy, it shall be considered a partial absence from classes which constitutes an unexcused absence. Student further agrees that such days will be considered absences as defined above;
Student agrees that he/she will attend classes every hour he/she is scheduled;
Student agrees that he/she shall not engage in or provoke any disruptions in the school building, on school ground or enroute to school and agrees that any such disruptions shall summarily result in suspension. Student further agrees that he/she shall waive all right of appeal for any disciplinary action taken by Auburndale High School to the local school board, the Department of Instruction and/or any circuit court action;
Student understands that he/she will be removed from the rolls and not allowed to attend school if he/she: 1. Violates the school rules; 2. Is habitually truant; 3. Violates this contract.
The student may be readmitted as per statute. School agrees that educational services shall be provided student;
Both parties agree that if any section or part of this contract is adjudged invalid by any court of competent jurisdiction, the remainder of this contract shall not be affected thereby;
The parties agree that this contract can only be modified by written instrument.
Parental signature specifies that parents agree with the emancipating of their son/daughter.
